This year we will require ALL COACHES AND ASSISTANTS to acquire their own background check forms. The forms are attached; you MUST complete the "Criminal Background Check" Form first. When that is returned to you, you will then need to complete the "Child Abuse" form and attach a copy of the criminal background form when sending in.

Each form will only cost $10.00. (A small price for the safety of our children)

These forms can take up to 4 weeks to be processed.
" NO ONE" WILL COACH IN THIS ORGANIZATION WITH OUT AN APPROVED BACKGROUND CHECK.
